2010/2011 STUDENT REGISTRATION
INFORMATION
PRESCHOOL - 8th GRADE
New Student Registration BEGINS JANUARY 25
7:30 AM – 3:00 PM in the school officeRegistration fee is $125 per child (Non-refundable) + $10 Auto Draft Fee
Priority for available spaces is as follows:
1st - St. Dominic Registered Parishioners
2nd - Catholic families/children from parishes without a school
3rd – Catholic families/children from other parishes
4th – Non-Catholic students
Age Requirements:
Preschool - child must be four on or before September 10th
Kindergarten - child must be five on or before September 10th
1st Grade - child must be six on or before September 10th
Documents needed to register NEW STUDENTS:
Certified, original birth certificate (Preschool, Kindergarten, 1st Grade) (souvenir certificates are not acceptable)
Baptismal certificate, if Catholic
Academic records (most recent report card, testing information (if applicable)
Social Security Number
State of Alabama Immunization Certificate will be needed before school begins
Copies of divorce decrees and/or custody judgments issued by the courts (if applicable)
Acceptance
1) Registration form completed and returned
2) All financial obligations have been met
3) Birth (Preschool, Kindergarten & 1st graders), and baptismal certificates received
4) Academic records evaluated (copy of most recent report card)
5) Interview with principal, if deemed necessary
6) Written acceptance (new students 1-8)

*To qualify as a Contributing Parishioner of St. Dominic Parish, you must contribute a minimum of $1000 annually and have an Intention Card on file with the parish office. Parishioners at other parishes should consult with their pastor.
